Living independently in one’s own home is something to which many older people aspire. Circumstances can waylay such aspirations, but this does not have to mean the end of independent living. Care in Midstream has identified the need for quality Home Care Nursing Assistance within Midstream.
Home Care Nursing Assistance Categories:
Day Care – Home Care Nursing Assistance for a day or part of a day, to provide spouses or caretakers with the opportunity to take a break, attend to important business or time to nourish their own health.
Short Term Care – Home Care Nursing Assistance for a pre-determined period, up to one month.
Long Term Care – Continuous Home Care Nursing Assistance for a long term with regular mini assessments to adjust the Care Plan.
The care journey starts with a Care Assessment that determines the level of care required and, after consultation with the resident/family, the Care Plan is implemented by the Home Care Nursing Supervisor and executed by qualified Carers. Weekly evaluations and regular mini assessments are conducted for quality control, and regular feedback is given to the resident/family to ensure that the appropriate care is provided. Retire@Midstream residents qualify for a free Care Assessment.
The Care Assessment is completed in the comfort of the resident’s home by the Home Care Nursing Supervisor and will determine the appropriate care required.

Home Care Nursing Assistance Services:
Primary Care – includes hygiene and personal grooming
Nursing Care – includes administration of pre-packed medication, pressure point and wound care, and monitoring vital signs
General Care – mobility assistance, exercise facilitation and reading
Secondary Care – basic housekeeping and home management
Care in Midstream offers a flexible fee structure based on an individual’s Care Plan.
